About the role
The incumbent in this position functions as the Medical Center's Liaison/Case Manager for Fresno VAMC's Healthcare for Homeless Veterans Program Department of Housing and Urban Development / VA Supportive Housing (HUD-VASH).
Responsibilities
- Overseeing all aspects of the HUD-VASH program.
- Reviewing agreements between Section 8, landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements.
- Performing site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.
- Verifying the Veteran's status, eligibility for VA medical care, and eligibility of program participation.
- Establishing a process for referrals and admissions to the HUD-VASH Program, ensuring that homeless Veterans are considered as primary participants for program acceptance.
- Collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
- Providing oversight of HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing.
- Cooking care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, linking, and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
- Complying with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager.
- Actively participating in the HUD-VASH Program conference calls and attending training offered by the HUD-VASH.
- Developing and administering local policy and SOP for the HUD-VASH Program. This includes maintaining policies and documentation as required for CARF and JCAHO accreditation.
Requirements
Open to current permanent Department of Veterans Affairs employees, current permanent Federal employees of other agencies and all US Citizens.
Qualifications
- Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
- Must be proficient in written and spoken English.
- Must be able to pass a background/security investigation.
- Must pass a pre-employment physical examination.
- Must participate in the seasonal influenza vaccination program.
