Senior Social Worker
About the role
The Community Nursing Home Program Senior Social Worker is part of an interdisciplinary team that oversees Veterans residing in contracted community nursing homes. This role involves completing assessments during routine visits to determine and correct unmet clinical needs and ensure Veteran and family satisfaction.
Responsibilities
- Assesses Veterans during routine visits to identify and address unmet clinical needs.
- Ensures Veteran and family satisfaction with the care provided.
- Participates in quality oversight, approving contracts for eligible Veterans.
Requirements
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Qualifications
- Licensed or certified by a state to independently practice social work at the master's degree level.
- Experience post advanced practice clinical licensure, with at least one year equivalent to the GS-11 grade level in a specialized area of social work practice.
- Advanced practice skills and judgment, including experience in specialized interventions and treatment modalities.
- Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area.
- Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area.
- Ability to expand clinical knowledge in the social work profession and write policies, procedures, and practice guidelines.
- Experience in a major program area such as Polytrauma Rehabilitation Center, Spinal Cord Injury Rehabilitation Center, or a national VHA referral center.
- Collaborates with the interdisciplinary treatment team to provide comprehensive health care services to Veterans, ensuring equity of access, service, and benefits.
- Serves on committees, work groups, and task forces at various levels.
Skills
Skilled in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations, including individual, group, and family counseling or psychotherapy and advanced level psychosocial and/or case management.
