About the role

The Family Health Bureau under the Department of Health's Public Health Division works to improve the health of women, children, and families in New Mexico through policy, surveillance, assurance, and direct services provided to reproductive age women, infants, children, adolescents and their families, including children and youth with special healthcare needs. The staff in the bureau work to bring federal funds to New Mexico and provide guidance and oversight of program services provided in the regional health offices.

This position provides a safety net and assurance services for Children and Youth with Special Healthcare needs with special dietary and nutritional requirements for Children's Medical Services (CMS) and other programs within the Public Health Division (PHD). Services and concerns are addressed on both an individual/family level and on a systems level. Interviews are anticipated to be conducted within two weeks of certified list.

Responsibilities

  • Supports program operations by meeting caseload goals through performing clinic or facilities duties, traveling to clinics to provide services, outreach to health fairs and other programs to increase caseload, and providing customer service to applicants/participants.
  • May provide intake/certification process of the applicant/participant including observation form, health assessment, breastfeeding women/infant evaluation, breastfeeding women/infant/child midpoint assessments, anthropometrics (heights/weights/hemoglobin), and immunization status by interviewing participant.
  • Facilitate Nutrition Education classes using "Facilitators of Change" – Participant Centered Services (PCS) design.
  • May provide clinical nutrition assessments or prescribe food packages.
  • Provides group and one-on-one nutrition education, including breastfeeding promotion and support responsive to the identified needs/interests of each participant.
  • Identifies the need for individual care plans and implements them for low-risk participants.
  • Refers participants to other health and social services and provides appropriate follow-up to referrals.
  • Documents nutrition services provided, including referral and follow-up to referrals.

Requirements

  • Bachelor's degree in a social, community, health, education, or nutritional field and two (2) years of experience as it pertains to the essential duties and responsibilities of the classification.
  • Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience.
  • Must possess and maintain a valid Driver's License.

Ideal Candidate

The ideal candidate has experience:

  • Fluent in both English and Spanish.
  • Assisting CMS clients with linkages to community-based nutrition service providers who work within systems that limit access and support to ongoing nutrition care.
  • Obesity prevention and the prevention of diabetes.
  • Working within the Medicaid and insurance structures to ensure children/youth with special healthcare needs' nutritional/dietetic needs are met.
  • Establishing community partnerships to work within systems and advocate for clients' access to nutrition services (screening, assessment, and ongoing nutrition care).

Working Conditions

  • Position will provide services primarily in communities, clinics, and auxiliary sites.
  • Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage with extended periods of office noise, bending, standing, and lifting.
  • Ability to lift up to 50 lbs.
  • Services are provided in a clinic with a diverse racial and ethnic population, many of whom do not speak English.
  • Travel to satellite offices, including overnight and weekend travel to attend meetings, training sessions, and special events, is required.

Pay

$27.37 - $41.06 hourly / $56,931 - $85,396 annually. This position is a Pay Band C7.
