Senior LTSS Service Care Manager

Responsibilities

• Complete initial assessments for children entering the STAR Kids program.
• Identify potential unmet needs and assess for Medicaid services beneficial to the child's condition.
• Conduct 6 member visits per week, typically 2 visits per day with 2 days allocated for documentation.
• Schedule visits at 3:00 pm, with potential to extend beyond 5:00 pm due to travel.
• Ensure timely submission of documentation as per specific turnaround times.
• Meet performance metrics based on documentation and training compliance, as well as company goals.

Requirements

• Education/Certification: RN, ADN, BSN, or MSN
• Licensure: Registered Nurse
• Minimum 3 years of nursing experience
• Minimum 2 years of case management experience
• Must be located within the service area
