About the role

We are seeking an experienced Registered Nurse (RN) to join a care management team providing services to individuals with long-term support needs. The role involves assessing patient needs, developing care plans, coordinating care among various healthcare providers, and ensuring compliance with regulatory standards.

Responsibilities

• Assessing patient needs and developing comprehensive care plans
• Collaborating with interdisciplinary teams to coordinate care
• Evaluating care effectiveness and making adjustments as necessary
• Maintaining accurate and up-to-date medical records
• Ensuring all care is compliant with relevant regulations and guidelines
• Communicating effectively with patients, families, and other healthcare professionals

Requirements

• Bachelor’s degree in Nursing (BSN)
• Current RN license in good standing
• At least 2 years of experience in long-term care or home health care
• Experience in care coordination and case management
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite

Qualifications
