Responsibilities
- Perform case management duties at an advanced level, demonstrating understanding of patient outcomes, hospital goals, and the department's impact.
- Analyze length of stay (LOS), costs, outliers, and potential discharge planning issues.
- Assign patient caseload to department members and self for optimal delivery of Case Management services.
- Act as an interdisciplinary team leader, collaborating with other departments, medical staff, and hospital leadership.
- Serve as a preceptor for new case management staff or students, assisting with onboarding, orientation, and training.
- Maintain knowledge of hospital performance improvement programs, CMS Conditions of Participation, Medicare Coverage Criteria, and relevant Joint Commission standards.
- Oversee department staff and operations in the absence of the Director of Case Management (DCM).
Requirements
- CCM or ACM certification required.
- For RN: Valid, unencumbered license in the state where the hospital is located or from an appropriate Nursing Licensure Compact state. Minimum of an associate degree in nursing (AND or ASN) required; bachelor's degree (BSN) preferred. A diploma in nursing is acceptable only in states where it is the minimum requirement for RN licensure.
- For licensed or certified health care professionals other than RN: If licensure is available in the state and applicable to the individual's educational level (e.g., BSW), a valid, active, unencumbered license is required. Minimum of a bachelor's degree in a health or human services-related field; master's degree preferred.
- Minimum of 2 years of experience in Case Management, including Utilization Review and Discharge Planning in an inpatient rehabilitation setting.
