Responsibilities
- Aid in outreach and engagement of residents at 2605 Sedgwick and Bronx Park Phases.
- Support NYSERDA Green Energy program - assist program participants and provide follow-up services.
- Directly serve clients, conducting intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries, and follow-up.
- Manage a caseload, monitor and document client progress towards service plan goals, and maintain familiarity with available program resources.
- Coordinate client services with other social service, mental health, medical, employment, educational, childcare, and other providers.
- Complete program reports on a monthly basis or more frequently as required.
- Respond to inquiries concerning clients and act as a program contact person.
- Collaborate and support securing benefits, particularly related to access to benefits services and eviction prevention efforts.
- Report to and meet with supervisor monthly.
- Conduct home visits and other field work as required.
- Facilitate monthly activities that align with the site's needs.
- Assist with community events as needed and serve as a liaison with community vendors.
Qualifications
- Bachelor’s degree with case management experience or High School Diploma with 3 years of case management experience.
- Experience using Access HRA and securing HRA entitlements and benefits preferred.
- Mental health background preferred.
- Bilingual preferred.
- Crisis prevention and mental health first aid training is required within the first 6 months.
- Strong oral and written communication, time management, and organizational skills are necessary.
- Proficiency in Microsoft Office suite and other standard business technology is required.
Physical Requirements
- Ability to use a computer for prolonged periods.
- Occasional lifting and/or moving up to 10 pounds.
- Standing, walking, or sitting for long periods of time, or climbing five flights of stairs, if required to conduct home visits and/or fieldwork.
- Bending and retrieving objects and/or documents.
- Traveling in the boroughs of New York City and its adjacent counties via public transportation.
- Traveling for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
