About the role

Join a team where your expertise provides comfort and guidance when patients and families need it most. As an Afterhours RN Care Manager, you'll deliver compassionate hospice care outside of traditional business hours, ensuring seamless, high-quality support throughout evenings, nights, weekends, and holidays.

Responsibilities

• Serve as the primary clinical resource for patients and families during after-hours coverage.
• Assess patient needs, coordinate care, and provide timely interventions to ensure continuity and quality of hospice services.
• Collaborate with physicians and the interdisciplinary team (IDG) to ensure coordinated, patient-centered care.
• Maintain accurate documentation and communication while providing clinical oversight for assigned patients.
• Support quality initiatives through clinical workflow reviews, documentation audits, and survey readiness activities.

About You

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Meet any state-specific requirements.

Skills And Competencies

• Strong knowledge of hospice principles, Medicare/Medicaid regulations, and best practices.
• Effective communication with patients, families, physicians, and interdisciplinary team members.
• Strong time-management and prioritization skills in a fast-changing environment.
• Proficiency with EMR systems (HomeCare HomeBase preferred), Microsoft Office, and mobile technologies.
• Leadership skills to support and guide field staff.
• Ability to manage administrative, documentation, and compliance responsibilities accurately.
