Primary Job Function

The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enable them to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Essential Job Function

  • Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations;
  • Coordinate all services throughout treatment;
  • Conducts initial assessment and evaluation to determine eligibility for services;
  • Complete Bio-psychosocial assessment;
  • Establish individualized treatment plan with each patient and review as clinically indicated;
  • Respond, as needed, to patient grievances and complaints;
  • Develop aftercare and discharge plans.

Qualifications

  • Must be an MLADC, LADC, Clinical Mental Health Counselor (LCMHC), or Clinical Social Worker (LCSW) in the state of New Hampshire;
  • Satisfactory drug screen and criminal background check;
  • Must demonstrate a positive, non-judgmental, and accepting attitude toward our patient population;
  • Ability to collaborate with compassionate and knowledgeable counseling team members.
