Responsibilities
- Meet caseload and/or billing expectations on a monthly, quarterly, and yearly basis.
- Maintain a consumer caseload as established with a designated supervisor, with composition determined by caseload intensity and regional service structure.
- Develop and monitor a Person-Centered Recovery Plan (PCRP) as required by established rules.
- Assist with gathering information on service availability and engagement statewide to support the service mapping process.
- Assist with training and orientation of employees within the SDMI Waiver Case Management program.
- Engage in community outreach and education as appropriate.
- Support the SDMI Waiver enrollment process, including assessing needs and confirming service availability for waitlist members approved for services.
- Assist members’ entry into services and monitor that needed services and benefits are received.
- Complete chart audits as directed and required by the program.
- Support service plan reviews and provide feedback to ensure compliance and alignment with policy.
- Assess clients, families, and environments, including conducting home visits and transporting persons in corporate and/or personal vehicles.
- Follow State of Montana and AWARE rules, policies, procedures, and guidelines.
- Prepare reports, applications, and supporting documentation within timelines established by the supervisor.
- Attend workshops, conferences, and meetings; participate in committees; and present information to other Case Managers and AWARE employees as requested.
- Utilize AWARE’s Core Values, Unconditional Care Principles, and wrap-around philosophy focused on client and family engagement.
- Attend mandatory training, in-services, staff meetings, and committees as assigned.
- Demonstrate consistent willingness and ability to productively utilize work time to meet client needs while meeting fiscal goals (billing, caseload maintenance).
- Record notes, billing, assessments, referrals, and other data entry in AWARE’s Electronic Healthcare Record System, Plan of Care, with sensitivity to age, gender, disability, sexual orientation/identity, racial, ethnic, and cultural differences.
- Perform other duties as assigned.
Requirements
- BS/BA Degree in a social work-related field.
- Minimum of one (1) year of direct work experience in adult mental health case management.
- One (1) year of experience in the human service field.
Skills
- Ability to develop and implement individual treatment plans with knowledge of behavioral, clinical, and mental health interventions.
- Strong interpersonal, oral, and written communication skills to work as a team member, facilitate meetings, and report observations accurately.
- Ability to learn behavioral, clinical, and mental health care methods and follow oral and written instructions.

Pay
$59,987.00 per year.
