School-Based Health Center Social Worker - LCSW/LMSW/LMHC or LMFT (10 Month/Summers Off )

About the role

The School-Based Health Social Worker provides Behavioral Health Services to patients within the School-Based Health Department managed by NCFHC. The Social Worker is involved in the evaluation and treatment of patients with behavioral health diagnoses and provides input and support for other clinical staff involved in the care of the patients.

Responsibilities

• Evaluate and treat patients with behavioral health diagnoses
• Provide input and support to other clinical staff involved in patient care

Qualifications

• New York State Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Master Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT)
• Excellent verbal, listening, and documentation skills
• Ability to maintain complete and accurate records for quality of care, treatment planning, and billing

Schedule

• School-based position following the school faculty calendar
• Paid school vacations, holidays, and snow days during the 10‑month school year
• Travel to school‑based locations required
