About the role

This position provides case management to children in Patrick County local schools, specifically Stuart Elementary, ensuring they receive necessary services to access medical, psychiatric, social, education, and other essential supports.

Responsibilities

The case manager will be responsible for ensuring that youth receive services to assist them in accessing the needed medical, psychiatric, social, education and other supports essential to meeting their basic needs.

Qualifications

• Bachelor's Degree in human services or related field is preferred
• One year experience of direct care or any equivalent combination of training, skills and abilities as defined by DMAS

Requirements

• Must be willing to consent to background checks, including checks for barrier crimes
• Must be willing to undergo drug screening as part of the application process

Salary is commensurate with education and experience as it relates to the position.
