Case Manager - School-Based

About the role

From participating in intakes to determine consumer needs and capabilities to serving as primary liaison between HCS and the school regarding HCS services and service provisions, this position gives you the opportunity for meaningful, hands-on work alongside community partners for greater continuity of care.

Responsibilities

• Participate in intakes to determine consumer needs and capabilities
• Serve as primary liaison between HCS and the school regarding HCS services and service provisions

Qualifications

• Bachelor's degree in Human Services or related field
• Registered as a Qualified Mental Health Professional and experience in a school setting preferred

Location

Washington County VA or City of Bristol, VA

Schedule

Full-time, Non-exempt, Monday-Friday

Compensation

$18.50 - $20.00 – Commensurate with education/experience
