Responsibilities

• Coordinate and manage schedules for patient visits, ensuring proper team assignments
• Record and communicate schedules with staff and clinical team members
• Support a collaborative, patient-focused environment
• Build strong working relationships with team members

Requirements

• High school diploma, or equivalent
• Two (2) years of work experience in home health required; Home Care Home Base experience preferred
• Excellent communication and organization skills
• Ability to multitask, ensuring the completion of several tasks, projects, and reports throughout the day
• Intermediate functional knowledge of computer/technology skills including, but not limited to MS Office, MS Excel, navigation of company website, and electronic medical record
