About the Role

As a Home Health - Clinical Scheduling Coordinator at Transition Home Healthcare, you will play a crucial role in ensuring our clinicians' schedules align with patient needs and authorizations. You will serve as the primary point of contact for scheduling-related interactions with both clinical and administrative teams, fostering positive relationships while maintaining efficiency and productivity.

Responsibilities

• Create, maintain, and update clinician schedules based on authorizations and patient needs.
• Maintain an up-to-date clinician availability list.
• Act as the point of contact for scheduling interactions with clinical and administrative teams.
• Maintain a tracking system to ensure proper scheduling of all visit frequencies.
• Collaborate with the Director of Nursing (DON) to ensure clinicians meet productivity goals.
• Assist with staff schedule changes and patient reassignments as needed.
• Maintain positive working relationships with internal and external customers, upholding the core values of Transition Home Healthcare.

Requirements

• 1+ years of experience with healthcare scheduling, specifically in Home Health.
• Bilingual (fluent in Spanish) is encouraged but not required.
• Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Experience with Home Care Home Base EMR is a plus.
