Ryan White Program Nurse (LPN)

About the Role

The Nurse Case Manager coordinates primary care for Ryan White patients under the direction of the Ryan White HIV/AIDS Part C Program Coordinator. This includes completing initial histories, patient education, linkage to care, and medication and treatment adherence.

Responsibilities

• Adhere to the essential duties and responsibilities of Community Health Net's Nurse Position.
• Provide case management services, including linking patients with primary medical care, psychological, and other support services to ensure timely, coordinated access to care and continuity of services.
• Initiate comprehensive assessments of patients' needs and personal support systems.
• Develop comprehensive, individualized service plans for patients and coordinate required services.
• Process positive HIV referrals, including intake information, initial physical exams, ordering lab work, scheduling referrals, and connecting patients to community service programs specializing in HIV care.
• Chart treatment plan changes and triage patient phone calls.
• Conduct confidential HIV testing and counseling.
• Perform phlebotomy and collect lab specimens.
• Assist with coordination of the Quality Improvement (QI) program to ensure compliance with clinical protocols.
• Help patients obtain HIV medication coverage through the Special Pharmaceutical Benefits Program (SPBP).
• Report all HIV positive tests to the Erie County Health Department.
• Educate patients on HIV prevention, coping strategies, and disease management, including physical, social, and psychological aspects.
• Provide education and support to loved ones and family members of those infected with HIV.
• Assist with entry of patient data into the clinical management information system.
• Participate in internal and external Ryan White meetings.
• Enhance professional growth through educational programs, seminars, and current literature.
• Perform other duties as assigned by the Ryan White Program Coordinator.

Requirements

• Excellent communication skills and ability to multitask.
• Proven confidentiality in handling patient and organizational information.
• PC knowledge, keyboarding skills, and familiarity with HIV medical terminology.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current Pennsylvania license.
• Minimum of two (2) years of clinical experience, including phlebotomy.
• Minimum of one (1) year of experience in case management, HIV, and/or drug and alcohol counseling preferred.
