Lpn Nurse
This Licensed Practical Nurse (LPN) position in the home care division focuses on providing comprehensive, one-on-one healthcare services to patients in their homes. Working under the supervision of a registered nurse or physician, you deliver direct patient care, administer medications and treatments, and support a wide range of healthcare procedures in accordance with established protocols and regulatory guidelines. You play a key role in promoting patient comfort, safety, and overall well-being while helping patients and families manage health conditions in a home setting.
Responsibilities
- Conduct comprehensive assessments of patients’ health status and document findings clearly and accurately.
- Administer medications, treatments, and therapies as prescribed by the physician or registered nurse, following all clinical protocols and safety standards.
- Monitor patients’ vital signs and promptly report any significant changes or abnormalities to the appropriate healthcare provider.
- Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and other personal care needs.
- Provide wound care, injections, and other skilled nursing services in accordance with established clinical guidelines.
- Educate patients and their families on health management techniques, proper medication administration, and disease prevention strategies to support long-term wellness.
- Maintain accurate, up-to-date patient records, including documentation of assessments, interventions, and outcomes, in compliance with regulatory and organizational standards.
- Collaborate effectively with other members of the healthcare team to ensure continuity of care and optimal patient outcomes.
- Adhere to all regulatory requirements, policies, and procedures governing home healthcare services, including infection control and patient safety protocols.
- Work independently in patients’ homes while maintaining professional boundaries, confidentiality, and a patient-centered approach to care.
Requirements
- Valid New York State licensure as a Licensed Practical Nurse (LPN).
- Current CPR certification.
- Strong clinical assessment and critical thinking skills to identify patient needs and respond appropriately.
- Excellent communication and interpersonal abilities to interact effectively with patients, families, and healthcare team members.
- Ability to work independently in a home care setting while also collaborating as part of a multidisciplinary team.
- Compassionate, patient-centered approach to care delivery, with a focus on dignity, respect, and comfort.
- Reliable transportation and willingness to travel to patients’ homes within the designated service area.
- Ability to follow established protocols, regulatory guidelines, and home care policies consistently.
Work Environment
This role is based in a home care setting, providing one-on-one nursing care in patients’ residences. You travel within a designated service area and work in varied home environments, adapting to each patient’s needs and living situation. The position involves close collaboration with remote or visiting registered nurses and physicians, as well as other members of the healthcare team. The work typically follows a structured schedule aligned with patient visits and care plans, with an emphasis on safety, professionalism, and respect for the patient’s home. You use standard nursing equipment and supplies appropriate for home care, and you maintain a professional appearance in accordance with home care dress standards.
Pay
The pay range for this position is $72,800.00 - $83,200.00 per year.
