Rockstar Director of Social Work SNF

Job Duties / Responsibilities

• Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.
• Performs resident evaluations as required by state regulations.
• Supports state regulations for admission and retention of residents.
• Aids residents and families with the move in process.
• Orients residents and families to the facility.
• Aids and interacts with residents and families to access services.
• Participates in development and implementation of each resident's individual service plan.
• Communicates with other members of the management team to ensure coordination of services to residents.
• Documents case management notes in residents' files as necessary.
• May perform other duties as assigned.
• Knowledge of MDS and discharge planning are essential.

Qualifications

• MSW
• Experience in the field of social work providing services to an adult population as required by state regulation if applicable.
• Two to five years’ experience in social work providing services to an adult population is preferred.
• Computer knowledge, particularly in Microsoft Word and Excel.
• Certified/Licensed as required by state regulations if applicable.
• Bilingual preferred

Pay

$100,000.00 - $120,000.00 per year
