About the role
Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary team members to coordinate care plans.
Responsibilities
- Administer medications and treatments as prescribed
- Assess patient’s condition and needs on a regular basis
- Provide emotional support to patients and families
- Coordinate with other healthcare providers to ensure comprehensive care
- Document all interactions and care provided accurately
Requirements
- Bachelor's degree in a related field
- Current CPR certification
- Experience in healthcare or related field preferred
Qualifications
- Strong communication skills
- Ability to work independently and as part of a team
- Passion for providing compassionate care
Skills
- Excellent interpersonal skills
- Proficient in computer systems used for documentation
- Knowledge of hospice care protocols and standards
