About the role

The Skin Integrity Nurse is responsible for overseeing and managing skin care and wound prevention programs within our long-term care facility. This role requires a current RN license and a strong understanding of the integumentary system, with a commitment to ongoing education.

Responsibilities

  • Complete weekly head-to-toe skin assessments and document findings in the Electronic Health Record (EHR).
  • Ensure appropriate treatments and interventions are in place for identified skin issues.
  • Collaborate with the Clinician or Wound Nurse Practitioner (NP) to obtain or discontinue orders as needed.
  • Update the Assistant Director of Nursing (ADON) and Director of Nursing (DON) on any new or worsened ulcers, as well as non-healing wounds.
  • Verify and update resident care plans and documentation for identified skin impairments.
  • Monitor skin integrity practices and procedures to ensure compliance with the facility’s Skin Integrity Treatment (SIT) program.
  • Demonstrate proper infection control practices.
  • Assist in evaluating and implementing recommendations from the Quarterly Quality Assurance (QA) meeting.
  • Ensure residents with non-compliance behaviors or high risk for pressure ulcers receive appropriate interventions.
  • Attend and participate in weekly SIT meetings.
  • Update residents’ responsible parties on wound progress.
  • Educate new and inexperienced nurse aides in proper methods of patient care.
  • Perform other duties as requested by the DON or ADON.

Requirements

  • Graduate of an accredited school of nursing with a current RN license in the state where the facility is located.
  • Possess and demonstrate leadership, organizational ability, and strong communication skills.
  • Ability to utilize excellent communication skills to relate to patients, families, and staff members.
  • Commitment to ongoing education to update knowledge and skills.
  • Flexibility with working schedule.
