RN Visiting Nurse Case Manager

About the role

Masonicare Home Health & Hospice is seeking a Registered Nurse Case Manager in the Danielson, CT area. This position involves ensuring quality and safe delivery of nursing services to patients in their homes. Visits are conducted primarily during the day shift, with occasional weekend rotations.

Responsibilities

• Develop, implement, and evaluate the plan of care for patients in the home setting.
• Coordinate the plan of care with other healthcare professionals to ensure continuity of care.
• Identify interdisciplinary care needs and recommend additional home health care services.
• Supervise the care provided by Licensed Practical Nurses and Home Health Aides, including conducting supervisory visits as required by regulations.
• Document patient and job information accurately and timely using the electronic system to support the OASIS data set, orders for service, time sheets, and other required documents and reports.

Requirements

• Minimum 1 year clinical experience is required.
• New graduates are not eligible.
• Previous home care experience is preferred but not mandatory.
• On-the-job training will be provided.
