RN - Vascular Access Team

About the role

The Staff Nurse delivers safe, compassionate care across diverse patient populations by developing, implementing, and evaluating individualized care plans. They collaborate with the healthcare team to ensure optimal patient outcomes and continuity of care.

Responsibilities

• Assess patients and gather health data from patients, families, and providers.
• Develop and revise care plans, documenting interventions and patient progress.
• Communicate care plans effectively to patients, families, and team members, respecting patient preferences and perceptions.
• Provide tailored patient and family education on disease management, emergency care, and medications, considering cultural and learning needs.
• Support professional development by sharing knowledge, mentoring colleagues, and engaging in performance improvement and research activities.
• Assist with technical procedures such as injections, venipuncture, IV management, and oxygen therapy.
• Participate in wound, ostomy, and skin care rounds, monitoring treatment effectiveness.
• Ensure compliance with HIPAA regulations and assist in patient transportation when necessary.

Qualifications

• Bachelor's Degree in Nursing required.
• Active Illinois RN license, ACLS, and BLS certifications required.
• Preferred experience in ICU, ER, radiology, or wound/ostomy nursing, with certification in wound care preferred.
• Strong communication, assessment, and collaborative skills.
