About the Role
Assist with all aspects of delivery of care and support for patients in an outpatient setting at Penn Medicine, a leading academic medical center dedicated to providing the highest level of care, conducting innovative research, and educating future leaders in medicine.
Responsibilities
- Assess, plan, implement, document, coordinate, and manage patient care according to department and hospital standards, considering age-specific accommodations.
- Maintain a computerized, evolving plan of care based on nursing assessment, medical plan, and patient needs.
- Utilize technology including eClinical Works, Meditech, and EMR systems to generate clinical reports and ensure documentation compliance.
- Provide patient care demonstrating knowledge of physiology, disease pathophysiology, and evidence-based interventions.
- Implement post-care based on evidence-based practice and document patient responses.
- Ensure accurate completion of charts, physician orders, and computer entries.
- Collaborate with office staff to complete procedure protocols and patient education.
- Avoid unapproved abbreviations in medical records.
- Intervene to prevent complications in collaboration with physicians.
- Evaluate, revise, and improve patient care using creative strategies and current research.
- Perform therapeutic interventions and diagnostic procedures, including:
- Medication administration and education (e.g., bridging anticoagulation therapy, new/adjusting medications, diuretic protocols).
- Initiation and maintenance of oxygen therapy.
- Obtain accurate vital signs, 12-lead ECG, and pulse-oximetry.
- Use 2 patient identifiers before interventions/procedures.
- Update accurate problem and medication lists.
- Assist patients physically and psychologically.
- Review lab results and manage medication holds for cardiology procedures.
- Manage Coumadin Clinic: adjust dosing based on INR results and determine retest dates.
- Maintain competency in emergency procedures and demonstrate clinical proficiency in crises.
- Communicate and collaborate with patients, physicians, family members, and healthcare disciplines to achieve excellence in patient care.
- Utilize effective interpersonal skills and involve patients/significant others in the plan of care.
- Communicate test results, instructions, and respond to inquiries.
- Share pertinent patient information during hand-offs or between departments using SBAR standards.
- Demonstrate leadership in team building and interpersonal relationships.
- Promote patient safety through adherence to protocols, policies, procedures, evidence-based guidelines, and OSHA goals.
- Analyze and prioritize incoming data for providers (e.g., open heart surgery reports, discharge instructions).
- Arrange services such as mobile lab, visiting nurse services, and follow-up visits.
- Respond to unusual clinical status by consulting healthcare team members and taking timely action.
- Maintain equipment and report failures; adhere to quality monitoring procedures.
- Safely handle drugs and body fluids using standard precautions.
- Actively participate in and support departmental and organizational quality initiatives and goals.
- Identify professional goals reflecting strengths and weaknesses and seek improvement.
- Attend and participate in meetings according to department standards.
- Support quality initiatives, including core measures, and review policies/procedures periodically.
- Maintain clinical competency according to unit-specific standards.
- Demonstrate competency in technological skills required for the role and adapt to age-specific patient needs.
- Maintain technological competence through practice, in-services, and professional conferences.
- Recognize legal responsibilities and functions within the Pennsylvania Nurse Practice Act.
Requirements
- Registered Nurse (RN) license in Pennsylvania (Required).
- Basic Cardiac Life Support (BCLS) certification (Required).
