About the Role

Fairview is seeking an experienced Registered Nurse (RN) Navigator to support our Gastroenterology (GI) team in a 100% remote role. This position provides care navigation, communication, and education to patients and family members through triaging patients after a referral to the clinic is placed. The role works closely with providers and the scheduling team to best meet patient needs and corresponds with referring providers, both internal and external, as needed.

This is a 0.8 FTE (64 hours per pay period) position with a schedule of Monday through Friday, 8:00 a.m. to 4:30 p.m., including one scheduled day off per week. There are no weekend, holiday, or on-call requirements.

Responsibilities

  • Navigates patient care from initial referral until consultation with the care team.
  • Coordinates care for patients new to the specialty.
  • Works with patients and families to prioritize patient needs and goals for initial appointments.
  • Anticipates future needs of the patient and educates and/or refers to appropriate sources.
  • Assesses patient clinical history and, if known, diagnosis, to create an initial plan for provider appointments and, if appropriate, further diagnostic tests.
  • Collaborates with schedulers to schedule initial appointment(s) and assists the scheduling team to resolve any barriers to carrying out the appointment plan.
  • Provides patient/family education regarding additional testing and work-up needed.
  • Develops and engages patient and family in the plan of care.
  • Works at the highest level of scope of practice and skills.
  • Documents and updates EHR as the initial appointment care plan is developed.
  • Follows standard work guides and workflows.
  • Collaborates with the records team to obtain notes, imaging, results, etc., from outside the health system.
  • Serves as a resource to referring offices regarding the specialty.
  • Maintains strong communication and relationships with the clinic team and referring offices.
  • Communicates with referring providers to gain additional clinical history and/or inform them of the initial appointment plan, including recommendations based on provider review of the patient.

Requirements

  • Associate Degree in Nursing or Bachelor’s Degree in Nursing.
  • 3 years of nursing experience within a specific patient population group.
  • MN Registered Nurse (RN) License.

Pay

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, including FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, the organization values pay equity and considers internal equity when making offers. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of the offer.
