About the role
Seeking a Registered Nurse (RN) to join the Transition team for full-time night shifts, providing skilled nursing care in patients' homes during a provisional period of 1–90 days. The team focuses on efficiently transitioning patients to home care while recruiting and training new full-time caregivers. Nurses assess, plan, implement, and evaluate patient care, monitor physiological and psychosocial changes, and document nursing diagnoses in accordance with prescribed orders.
Responsibilities
- Utilize the nursing process for patient assessment and care delivery
- Provide skilled nursing services during home assignments
- Assess changes in patient condition and document findings
- Collaborate with healthcare team members to ensure quality care
Qualifications & Skills
- Licensed as an LPN/LVN in the practicing state
- Current TB or Chest X-Ray and BLS certification
- One year of nursing experience, with tracheostomy and ventilator experience preferred
Key Duties
Deliver safe, comprehensive nursing care, support patient stabilization, and facilitate transition to full-time home care staff.
