About the role

Under the supervision of the Care Coordinator Manager/Supervisor, the Primary Care role performs tasks and patient contact to assist in the coordination of patient primary care services. This includes obtaining and running reports to assist with review and resolution of care gaps.

Responsibilities

  • Reviews monthly Care Gap data from the Managed Care Department for Family Medicine and General Medicine clinics.
  • Performs Transitional Care Management (TCM) non-face-to-face patient visits per policy and procedure.
  • Contacts insurance Case Managers under the direction of the Care Coordinator Manager/Supervisor and implements plans to follow up with patients, including phone calls and video discussions to assist in preventing or reducing ED visits.
  • Reviews Healthy Planet Performance of each clinic monthly, identifies measures that require improvement, and communicates findings to the Care Coordinator Manager/Supervisor.
  • Maintains a positive, customer-service-oriented disposition toward all clients and co-workers.
  • Provides excellent customer service to internal and external customers.
  • Prepares and maintains all clerical aspects of medical records through established guidelines.
  • Performs all other duties as assigned by management within job scope.

Requirements

  • 1 year of experience in an ambulatory primary care setting (required).
  • 2 years of experience in an ambulatory primary care setting (preferred).

Qualifications

  • Associate degree in Nursing (required).
  • Bachelor’s degree in Nursing (preferred).
  • 5 years of relevant work experience (required).
  • Registered Nurse (RN) with Florida licensure (required at time of hire).

Additional Details

  • Travel: Up to 10%.
