About the Role
As a Care Transition Liaison - RN (CTL-RN) with AmeriMed Infusion Pharmacy, you will play a key role in providing coordinated home health services for referred patients in the West of Cincinnati and Lawrenceburg, Indiana coverage area. You’ll collaborate with healthcare professionals, identify patient needs, and facilitate seamless transitions to home health care.
Responsibilities
- Collaborate with hospitals, nursing homes, and home health personnel to ensure quality care.
- Meet or exceed established monthly admission target(s) as provided by your supervisor and determined by the branch monthly budget.
- Coordinate patient referrals, evaluate eligibility for services, and assist in arranging home health care.
- Analyze patient needs and educate caregivers about home health options, Medicare/Medicaid guidelines, and insurance benefits.
- Develop relationships with healthcare providers to promote home health services and best practices.
- Participate in provider education and support hospital staff as a home health resource.
- Conduct trend analysis and identify opportunities for enhancing home health service outcomes.
- Contribute to Quality Assurance Performance Improvement (QAPI) initiatives.
Requirements
- RN license in the state(s) served (Ohio and Indiana).
- Minimum of 2 years of healthcare experience, preferably in home health or sales/marketing.
- Strong communication, assessment, and decision-making skills.
- Ability to build relationships, educate, and promote services effectively.
- Committed to professional development and improving own skills.
Pay
$37.40 - $55.63 per hour, plus mileage reimbursement at 58.5 cents per mile.
