About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement, and ensuring these services align with the interdisciplinary plan of care.

Responsibilities

  • Supervise, evaluate, and coordinate the interdisciplinary team, ensuring continuity of care from admission to discharge or transfer to bereavement.
  • Serve as patient advocate and coordinator for social service and health care providers.
  • Maintain patient records from intake through discharge or transfer to bereavement, ensuring eligibility and appropriateness criteria are met.
  • Collaborate with team physician and attending physician on palliative care measures for pain and symptom management.
  • Review imminent “alive” discharges to ensure effective discharge planning.
  • Monitor and manage team services, ensuring they are appropriate and responsive to patient/family needs and expectations.
  • Facilitate team meetings, ensuring interdisciplinary input into the plan of care.
  • Manage team schedule, coordinating services 24/7.
  • Assure staff competence and performance through field visits, evaluations, and in-services.
  • Oversee staff and volunteer schedules, ensuring equitable workload distribution and meeting productivity expectations.
  • Review and approve payroll, ensuring accurate time, mileage, and expenses.
  • Ensure clinical records and team activities meet regulatory standards and participate in quality improvement activities.
  • Complete and implement patient/family satisfaction surveys and quality improvement measures.
  • Perform utilization reviews and assist with regulatory body requests.
  • Address customer service issues and resolve problems promptly and satisfactorily.
  • Visually inspect LTC and Contract Bed facilities to ensure care plan integration, customer satisfaction, and appropriateness.
  • Participate in professional development activities and mentoring of newly hired Team Managers.

Qualifications

  • Reliable transportation with appropriate license and insurance coverage.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care principles, with a working knowledge of oncologic nursing and palliative care.
  • Understanding of interdisciplinary health care team dynamics and the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and ability to utilize management reports.
  • Two years successful supervisory experience or equivalent in a health care organization.
  • Bachelor's degree preferred.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.
