Clinical Team Manager (RN) - Hospice Home Care

About the role

Manages and supports an interdisciplinary team delivering hospice care, ensuring compliance with regulatory standards and clinical quality initiatives. Demonstrates excellence in clinical practice and customer service under general supervision.

Responsibilities

• Coordinate comprehensive patient assessments addressing medical, physical, social, emotional, and spiritual needs.
• Provide support and guidance to patients’ families aligned with the interdisciplinary plan of care.
• Participate in team meetings, committees, and quality improvement activities to enhance care delivery and compliance.
• Ensure adherence to hospice regulations through audits and monitoring processes.
• Contribute to the development and implementation of quality and educational initiatives.
• Assist in clinical, regulatory, and fiscal practice analysis to meet organizational goals.
• Oversee daily operations, including caseload management, productivity, and fiscal targets.
• Perform managerial duties such as staff training, performance evaluation, hiring, and budget contributions.
• Manage the interdisciplinary team to deliver effective hospice services to patients and families.
• Engage in special projects and perform additional duties as assigned.

Qualifications

• Current NY State RN license; Nurse Practitioner registration preferred.
• Bachelor’s Degree in Nursing or related field; clinical management experience preferred.
• Minimum three years hospice clinical experience; strong communication skills required.
• Knowledge of computer operations and MS Office preferred.
