Clinical Team Manager (RN) - Hospice Home Care

Responsibilities

• Assess patients' medical, physical, social, emotional, and spiritual needs using an interdisciplinary approach.
• Provide support and guidance to families in line with the hospice care plan and policies.
• Participate in team meetings, management discussions, and committees to promote collaboration, develop procedures, and enhance quality and training.
• Ensure regulatory compliance through audits and reviews of files and processes.
• Contribute to quality improvement and educational initiatives to maintain professional standards.
• Support achievement of hospice care goals, including managing operations, caseloads, and productivity targets.
• Lead and facilitate interdisciplinary team efforts to deliver comprehensive hospice services.
• Perform managerial duties such as staff training, performance evaluation, budgeting, hiring, and disciplinary actions.
• Perform other assigned duties as needed.

Qualifications

• Licensed Registered Nurse and Nurse Practitioner in New York State preferred, with a Bachelor's in Nursing or equivalent experience.
• Minimum of three years hospice clinical experience required, with clinical management skills preferred.
• Effective communication skills and proficiency in MS Office are desirable.

Pay

• $15K sign-on bonus, payable in installments.
