About the role

In absence of Director or when necessary, assists the Director to plan, direct, supervise, and monitor the activities of the department to achieve optimum patient care by meeting established goals and objectives. Coordinates these activities with the Medical Staff, Administration, families, ancillary services, and community agencies to enhance total patient care.

Responsibilities

  • Maintain working relationships with patients, families, staff, physicians, and referral sources.
  • Take phone calls and make appropriate documentation of these calls.
  • Obtain orders from physicians as needed.
  • Aids in critical lab reporting/monitoring.
  • Communicates patient care problems or potential problems to the Director to assure continuity and coordination of care.
  • Assumes on-call responsibility per policy/procedure.
  • Performs essential functions of the RN.
  • Documents patient status and care or services furnished in appropriate areas, as defined by the documentation policy and procedure.
  • Maintains supervisory reports for LPH/HHA Compliance.
  • Completes and documents full system assessment, including physical and psychosocial aspects and identifies age-specific criteria upon admission and each visit thereafter.
  • Maintains supervisory reports for LPH/HHA Compliance.
  • Maintains on call and holiday schedule.

Requirements

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Valid driver’s license.
  • CPR certification within 30 days of hire date.

Qualifications

  • Bachelor's of Science in Nursing (BSN).
  • Two (2) years of previous experience in a hospital/home care/hospice leadership role preferred (such as charge nurse, or documentable consistent demonstration of strong leadership skills while functioning as a staff nurse).

Skills

  • The ability to perform concentrated and complex mental activity, critical thinking with frequent involvement in complex and highly-technical situations.
  • The ability to work successfully under highly-stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.
  • The ability to make sound, independent judgments based on scientific principles, and also be able to collaborate with other multi-disciplinary team members in an appropriate fashion.
  • The ability to comprehend and perform oral and written instructions and procedures.
  • The ability to calculate drug dosages and infusion stats.
