Part-time position, 7pm–7am every other weekend (Saturday/Sunday). Pay is negotiable.
About the role
We provide skilled nursing care to people recently discharged from the hospital, needing therapy, and to those requiring long-term care.
Responsibilities
- Assist in implementing and revising care plans, monitor health conditions, supplement direct care needs, and respond to emergencies or changes in status.
- Observe care assignments, manage coverage needs, evaluate staff performance, educate, and lead staff.
- Communicate changes in resident conditions to families and providers; collaborate with the interdisciplinary team.
- Review and investigate incidents, transcribe orders, audit medical charges, and maintain record compliance for accuracy.
- Explain health status, changes, and care expectations to families, residents, and staff; guide and direct care skills with intervention as needed.
Requirements
- Licensed to work as a Registered Nurse (RN) in the State of Connecticut.
- Prior long-term care experience required.
- Able to pass background check, drug screen, professional references check, and physical evaluation.
- Excels in leadership and clinical competency.
