RN SENIOR CARE - PRN

Position Summary

The Registered Nurse under the supervision of the department of nursing bears responsibility and accountability for the nursing care. The role involves performing comprehensive assessments, implementing and documenting nursing interventions, consulting with healthcare providers, attending treatment team meetings, conducting education sessions, and coordinating care in cases of medical or psychiatric emergencies.

Essential Functions

• Performs comprehensive admission assessment to obtain presenting complaints, history, and listing all medications.
• Prepares patient’s Medication Administration Record (MAR) listing all allergies and notes response to medications.
• Develops a nursing plan of care, implements and documents nursing interventions that are part of the Multi-Disciplinary Treatment Plan.
• Consults with Medical Director/Provider, Psychiatrist, Therapists, and other Healthcare providers in Multi-Disciplinary Treatment Plan.
• Attends Treatment Team meetings to discuss and update Nursing Care Plan section of Multi-Disciplinary Treatment Plan.
• Initiates and conducts individual and group nursing education sessions.
• Checks for consents and assists patient in understanding their diagnosis and accepting need for treatment.
• Communicates relevant clinical and other client care information to Program Director (or organization’s equivalent), Medical Director/Provider, and Treatment Team members as necessary and appropriate.
• Maintains ongoing monitoring of clients' health status to identify health needs, problems, and/or symptoms.
• Provides prompt, efficient, and responsive patient care.
• Coordinates and documents clients’ care in cases of medical or psychiatric emergency, providing a report to the Emergency Department or other appropriate resource.
• Educates clients regarding bowel activity patterns to maintain regularity, prevent constipation and impaction.
• Makes observations and records patient behaviors.
• Participates in the completion of maintenance of clients’ medical records.
• Maintains the environment of care to ensure a safe and secure area for patient, staff, and visitors.
• Participates in unit QAPI activities per leadership request.
• Provides patient, family, and caregiver’s education on health-related topics.
• Aids clients with personal care needs (bathroom, etc.) as needed or requested.
• Coordinates clients’ meals and transportation as requested or in absence of Unit Clerk.
• Completes other duties as assigned by Program Director (or organization’s equivalent).

Position Requirements

• Graduate of an accredited College/School of Nursing, with an Associate or Bachelor’s degree.
• Current state licensure as RN.
• Certification in gerontological or psychiatric nursing desirable.
• Acute-care clinical and/or psychiatric nursing experience required.
• CPR Certification, CPI Certification, First Aid Certification.
• Knowledge of community resources.
• Microsoft knowledge/Computer proficiency.

• Hours: PRN Monday- Friday
• Pay Range: $23.855/hour to $38.168/hour (rate of pay is based on applicable years of experience)
