RN,Registered Nurse-ONSITE Virtual
Purpose
Join our dedicated team at MercyOne Des Moines Medical Center, where RNs have a unique opportunity to deliver patient care through virtual platforms. In this role, you will assess and care for patients using video conferencing technology, ensuring seamless admissions, discharges, and patient education within our acute care unit.
What You Will Do
- Collaborate, communicate, and facilitate the optimal provision of care through a computerized &/or digital platform supporting daily management of care.
- Absess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
- Utilize clinical knowledge, critical thinking skills, and the principles of case management & adult education to coordinate, implement, and support the plan of care.
- Ensure a seamless, effective, and efficient transition of care across the continuum.
- Maintain quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
- Perform the roles: mentor, clinician, educator, consultant/collaborator, and navigator.
Minimum Qualifications
- Current Iowa RN license
- BSN preferred
- Minimum five (5) years of equivalent experience in health condition management in an adult acute care clinical setting with direct patient care experience within the last 10 years
