About the role

The Registered Nurse Navigator facilitates the patient journey to ensure proper navigation throughout the healthcare system upon discharge. The RN Navigator supports staff in coordinating care as patients transition through the healthcare continuum in outpatient settings. This role serves as a clinical resource to scheduling coordinators, assists in identifying appropriate services or providers for referrals, recommends alternative care locations when needed, and identifies discharge needs and services.

Responsibilities

  • Facilitate patient navigation through the healthcare system post-discharge.
  • Support staff in coordinating care during patient transitions across the healthcare continuum.
  • Serve as a clinical resource to scheduling coordinators.
  • Identify appropriate services or providers for patient referrals.
  • Recommend alternative locations of care when appropriate.
  • Identify discharge needs and required services.

Requirements

  • Currently licensed to practice as a Registered Nurse in the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
  • AHA BLS certification (prior to the end of the orientation period).
  • Two (2) years of nursing experience required.

Preferred Qualifications

  • Previous navigator work experience preferred.

Schedule

  • Full Time (0.90 FTE).
  • Varied shift hours.
  • Every other Sunday.
