About the role
As a RN Registered Nurse MDS Coordinator, you will play a vital role in ensuring the smooth operation of our facility's care coordination processes.
Responsibilities
- Coordinating the preparation and completion of minimum data set (MDS) assessments for residents
- Developing and implementing individualized care plans in collaboration with physicians, nurses, and other healthcare professionals
- Monitoring and documenting resident progress, identifying areas for improvement, and making necessary adjustments to care plans
- Providing education and training to staff members on MDS and care planning processes
- Maintaining accurate and detailed records of MDS assessments and care plans
- Collaborating with other departments to ensure comprehensive and coordinated care
- Participating in quality improvement initiatives to enhance resident outcomes and satisfaction
Requirements
- Current RN licensure in the State of New York
- Prior experience in a long-term care or sub-acute setting in a similar role
- Strong clinical skills and knowledge of chronic care management
- Excellent communication and interpersonal skills
- Able to work effectively in a fast-paced environment with changing priorities
- Strong attention to detail and organizational skills
