RN Registered Nurse MDS Coordinator

About the role

As a RN Registered Nurse MDS Coordinator, you will play a critical role in the clinical documentation and assessment process.

Responsibilities

• Collaborate with the interdisciplinary care team to ensure accurate and timely completion of Minimum Data Set (MDS) assessments and care plans
• Conduct resident interviews and gather information to inform MDS assessments and care plans
• Maintain accurate and up-to-date records of resident assessments and care plans
• Provide education and support to the care team members on MDS-related documentation and assessment processes
• Participate in quality improvement initiatives to ensure effective and efficient MDS processes

Requirements

• Current licensure as a Registered Nurse in the State of New York
• Prior experience in a long-term care setting, preferably as an MDS Coordinator
• Strong communication and interpersonal skills to effectively collaborate with the care team
• Proficiency in electronic medical records and MDS software
• Able to maintain confidentiality and handle sensitive information
