Job Summary

  • Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs.
  • Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.
  • Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress.
  • Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home.
  • Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.
  • Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.
  • Documentation: Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses.
  • Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.

Qualifications

  • Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university
  • Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse
  • Minimum 1 year community health nursing experience or equivalent professional experience
  • Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS
  • Complies with accepted professional standards and practice
