About the role
Provide compassionate and skilled care to patients in their homes. Assist with daily activities, medication management, and monitor health conditions.
Responsibilities
- Visit patients in their homes to provide necessary medical care and support.
- Administer medications as prescribed by healthcare providers.
- Monitor patient health and report changes to healthcare providers.
- Assist with daily living activities such as bathing, dressing, and meal preparation.
- Document patient visits and care provided in electronic health records.
Requirements
- High school diploma or equivalent.
- Current certification as a Registered Nurse (RN).
- Valid driver’s license and reliable transportation.
- Experience in home health care or related field preferred.
Qualifications
- Excellent communication skills.
- Ability to work independently and manage time effectively.
- Passion for providing quality care to patients in their homes.
