PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Registered Nurse | Clinic
- Delivers comprehensive, participant-centered care to older adults with complex medical needs.
- Works as part of an interdisciplinary team, providing direct nursing care, coordinating treatments, monitoring participants' health, and preventing unnecessary hospitalizations.
- Assists in educating participants and caregivers on disease management, medications, and healthy living.
- Maintains accurate clinical documentation, ensures compliance with regulatory standards, coordinates specialty appointments and follow-up care, and advocates for participants.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of Kansas.
- Graduate of an accredited school of nursing.
- Minimum of 1-2 years of clinical nursing experience, preferably in geriatrics, primary care, long-term care, home health, hospice, or a similar setting.
- Current BLS (Basic Life Support) certification.
- Strong clinical assessment, critical thinking, and care coordination skills.
- Excellent communication and interpersonal skills with the ability to build trusting relationships with older adults, caregivers, and interdisciplinary team members.
- Able to prioritize multiple responsibilities in a fast-paced clinical environment.
- Proficiency with electronic health records (EHR) and basic computer applications.
- Valid driver's license, reliable transportation, and an acceptable driving record.
Preferred Qualifications
- Bachelor's degree in Nursing (BSN).
- Experience working in a PACE program or with frail older adults and individuals with multiple chronic conditions.
- Experience with interdisciplinary team-based care and care planning.
- Knowledge of Medicare, Medicaid, and PACE regulations.
- Experience providing patient and caregiver education related to chronic disease management and preventive care.
