Hospice Home Care

Provides compassionate, comprehensive support for individuals facing terminal illnesses, helping them live with comfort, dignity, and peace in the place they call home. The program focuses on improving quality of life rather than prolonging it, addressing not only physical symptoms but also emotional, spiritual, and social needs.

  • A dedicated team of professionals—including nurses, aides, social workers, chaplains, and volunteers—works closely with patients and their families to create personalized care plans, manage pain and symptoms, and offer counseling and respite services.
  • Extends bereavement support to families after a loved one’s passing, ensuring ongoing care and guidance during their time of grief.

RN Case Manager

In the Midland Care Connection hospice program, an RN Case Manager plays a critical role in coordinating and delivering high quality, patient centered end of life care. They are responsible for assessing patients’ medical needs, developing individualized care plans, and ensuring that patients receive appropriate symptom management, medications, and therapies to maintain comfort and dignity.

  • Serve as the primary clinical liaison between patients, families, and the interdisciplinary hospice team, which may include physicians, social workers, chaplains, and volunteers.
  • Provide education and emotional support to families, help navigate complex medical decisions, and ensure compliance with regulatory standards, all while advocating for the patient’s wishes and enhancing quality of life during hospice care.

Pay

$3,000 sign-on bonus to be paid in two increments at 6 months and a year of employment.

Qualifications

  • Current and valid Registered Nurse (RN) license in the state of practice
  • Minimum of 2 years of nursing experience, preferably in acute care, home health, or hospice
  • Strong assessment and clinical decision-making skills
  • Knowledge of end-of-life care, symptom management, and palliative care principles
  • Excellent communication and interpersonal skills for interacting with patients, families, and interdisciplinary teams
  • Ability to work independently and manage a caseload effectively
  • Basic computer proficiency for electronic medical records and documentation

Preferred Qualifications

  • Hospice or palliative care certification (CHPN or equivalent)
  • Experience in case management or care coordination
  • Familiarity with regulatory standards and compliance for hospice care
  • Experience providing patient and family education on end-of-life issues
  • Strong organizational skills with the ability to prioritize complex tasks
  • Culturally sensitive approach to patient care
