Responsibilities
- Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs.
- Provides a complete physical assessment and history of current and previous illness(es).
- Completes initial evaluations, consents, and admissions for patients.
- Coordinates patient care for assigned caseload.
- Utilizes the nursing process to provide professional nursing care.
- Assesses and evaluates patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
- Maintains a weekly visit minimum as established by Office Administrator.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Works closely with patients on a 1-1 level but gets strong support from a patient care team.
- Can work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Must possess time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns).
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area.
