RN - Registered Nurse - Case Manager - Days
About the role
Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. Creates plan for care across the continuum, integrating patient/family preferences and values. Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records. Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

Requirements
- 1 year Case Management experience
- 1 year Acute Care bedside experience
- 1 year Managed Care Prior Authorization experience
- 1 year Utilization Management/Review experience
- CCM, ACM, or ANCC certification
Qualifications
Diploma from an accredited school/college of nursing and professional licensure at time of hire required. Licenses / Certifications / Registrations: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Monday-Friday 8am-4:30pm with rotating weekends every 9-10 weeks
