8:00 AM–4:30 PM, Monday–Friday with weekend rotation every 8–10 weeks and holiday rotation required.

Responsibilities
- Coordinates the overall interdisciplinary plan of care for patients, from admission to discharge.
- Creates a plan for care across the continuum, integrating patient and family preferences and values.
- Monitors patient medical necessity and level of care through assessments, ongoing evaluations, and patient records.
- Advocates for resources and removal of barriers.
- Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of the health plan.
- Acts as a resource for adequate medical record documentation, level-of-care recommendations, and services related to diagnoses and treatment options for post-discharge care.
Requirements
- BLS Provider certification obtained within 1 month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
- Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing.
- Diploma from an accredited school/college of nursing and professional licensure at time of hire.
