Schedule
- Full time, day shift (8 am – 4:30 pm)
- 5 x 8-hour shifts per week
- 4 individual weekend shifts required every 6 weeks
- One minor and one major holiday per year
Responsibilities
- Actively participate in multi-disciplinary rounds to review changes in patient status, progression, level of care, and discharge plans for all assigned patients. Identify resources necessary at discharge and ensure timely transitions, escalating care delays to leadership as appropriate.
- Communicate with and educate patients and families regarding emotional, social, and financial impacts of illness. Mobilize family/community resources to meet identified needs while advocating for patient and family empowerment in healthcare decisions and service access.
- Assess readmitted patients for perceived reasons for readmission.
- Organize and facilitate patient and family care conferences with the multidisciplinary team.
- Document discharge planning evaluation, ongoing assessment, discharge plans, multi-disciplinary rounds (MDRs), barriers to progression of care, avoidable days, and patient and family needs according to standard work.
- Communicate with payors and post-acute care services/facilities to ensure continuity of care, complete medical records, and accurate discharge reconciliation.

Requirements
- Associate of Nursing (Required)
- Bachelor of Nursing (Preferred)
- 2+ years of medical/hospital nursing experience (Required)
- Prior Care Management/Utilization Management experience (Preferred)
- Registered Nurse (RN) license (Required)
- Basic Life Support (BLS) certification (Preferred)
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) (Preferred)

$32.76 - $57.47 per hour
