RN - Registered Nurse

Role & Responsibilities

• Serves as a resource to team members by coordinating the use of resources and providing support to resolve issues escalated by LPNs on shift.
• Conducts clinical assessments for the identification, evaluation, coordination, and management of members' needs, including physical and behavioral health, social services, and long-term services.
• Identifies members for high-risk complications and coordinates care in conjunction with the member and healthcare team.
• Performs assigned waived testing in the laboratory to provide data for the diagnosis, treatment, and prevention of diseases.
• Receives specimens and stocks supplies when received in the department. Troubleshoots/resolves instrument problems and documents corrective actions in appropriate log and when necessary.
• Affirms quality control is properly performed and logged and follows proper department procedures for corrective action.
• Demonstrates the knowledge and skills to train new employees.
• Draws blood and administers injections as needed.
• Rotates weekends and participates in on-call duties as defined by the on-call policy.
• Collaborates with the nurse manager to create an environment of teamwork that supports improved outcomes and service.
• Supports the efforts of the facility to improve engagement by attending required/mandatory meetings.
• Conducts new admission assessments and completes new admission tasks within time frames specified by licensing regulations.
• Assists with admission and discharge processes to ensure efficient throughput and high-quality, patient-centered care.
• Makes appointments, maintains and edits shared calendars, processes paperwork, and ensures necessary follow-ups are complete, thorough documentation, performs and responds to audits to ensure health needs are being met in accordance with licensing regulations, effectively communicates with all relevant members of the care team (including residents, guardians, placement agencies, YFT treatment team members, and outside medical offices and entities), and other tasks as assigned.
• Maintains multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Collaborates and communicates with the Medical Director and interdisciplinary team regarding health needs of residents (including residents, guardians, placement agencies, YFT staff-medical/nursing, case management, clinical, executive, quality assurance, food services, intake and admissions, and campus operations, and outside medical offices and entities) while maintaining appropriate discretion and compliance with HIPAA.
• Completes and obtains necessary paperwork on each resident, schedules appointments that are needed within the set licensing parameters, reviews and enters any new or returning paperwork of all residents.
• Implements treatment care plans by facilitating authorizations/referrals within benefits structure or extra-contractual arrangements, as permissible.
• Manages illnesses and/or disabilities ensuring cost-effective and efficient utilization of health benefits; conducts gap in care management for quality programs.
• Manages and keeps all active prescriptions, ensures resident’s medications are available as appropriate, ensures medication orders are active and accurate in eMAR system, administers medication according to MATY protocol and YFT policy, completes medication administration documentation, monitors for side effects, performs medication audits, educates and supervises med techs and other medication management duties as assigned.
• Maintains and assists with managing medication expiration logs and supplies of each clinic/house/school. Ensures all supplies are available and not expired. Ensures all AEDs are tested according to policy and ensures all epi pen stations are stocked.
• Supports proper inventory control and assists with managing supplies and equipment.
• Participates in onboarding and development of nursing staff.
• Plans, coordinates, and conducts in-service programs designed for residents and staff to improve the quality of health care.
• Attends all training mandated by YFT and maintenance of license and certifications.
• Certified MATY Instructors: Trains all med techs according to MATY; schedules classes according to need and at discretion of the nurse manager.

Qualifications

• Current active license in Virginia as a Registered Nurse.
• CPR, First Aid, and BLS.
• Experience in adolescent, psychiatric, and/or case management nursing preferred.
• Bi-lingual (Spanish & English) required.
• Tech-savvy and familiar with electronic medical record systems.
• Excellent social, organizational, and communication skills required.
• Ability to apply professional knowledge to exercise independent judgement and discretion.
