Responsibilities

• Provide direct patient care to our residents, including assessing, planning, implementing, and evaluating individualized care plans
• Collaborate with our interdisciplinary team to ensure seamless communication and coordination of care
• Develop and implement effective care protocols to promote optimal patient outcomes
• Utilize our electronic medical record system to document patient information and track progress
• Participate in ongoing education and training to stay current with best practices and industry developments
• Provide skilled nursing care, including wound care, ostomy care, and medication management
• Conduct initial and ongoing assessments to identify care needs and develop individualized care plans
• Implement and monitor care plans, making adjustments as necessary to ensure optimal outcomes
• Communicate effectively with residents, families, and interdisciplinary team members to ensure coordinated care
• Participate in quality improvement initiatives to drive better patient outcomes and resident satisfaction
