Hospice Program
The hospice program at Midland Care Connection is designed to support individuals with life-limiting illnesses and their families by focusing on comfort, dignity, and quality of life. It includes personalized pain and symptom management, emotional and spiritual support, and practical assistance.
Responsibilities
- Completes the initial assessment by reviewing the patient’s medical history, evaluating symptoms, and determining eligibility for hospice services.
- Explains the hospice philosophy, services, and care plan to the patient and family, answering questions and obtaining necessary consents.
- Collaborates with physicians and the hospice care team to develop an individualized plan of care.
- Ensures medications and equipment are arranged.
- Helps families feel prepared and supported during the transition to hospice care.
Qualifications
- Active Registered Nurse (RN) license.
- Graduate of an accredited nursing program (ADN or BSN).
- Valid driver’s license with reliable transportation.
- Current CPR certification.
- Strong clinical assessment and documentation skills.
- Able to work independently and manage time effectively.
- Excellent communication and interpersonal skills.
- Able to provide compassionate care to patients and families.
Preferred Qualifications
- Previous hospice, palliative care, or home health nursing experience.
- Bachelor of Science in Nursing (BSN).
- Experience with admissions, care coordination, or case management.
- Familiarity with electronic medical records (EMR) systems.
- Knowledge of end-of-life symptom management.
- Certification in hospice and palliative nursing (CHPN) or willingness to obtain.

About the Role
Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
