Hospice Program
The hospice program at Midland Care Connection offers compassionate, nonprofit end-of-life care designed to support individuals with life-limiting illnesses and their families. Services include personalized pain and symptom management, emotional and spiritual support, and practical assistance.
Responsibilities
- Completes initial assessments by reviewing medical history, evaluating symptoms, and determining eligibility for hospice services.
- Explains the hospice philosophy, services, and care plan to patients and families, answering questions and obtaining necessary consents.
- Collaborates with physicians and the hospice care team to develop an individualized plan of care.
- Ensures medications and equipment are arranged.
- Helps families feel prepared and supported during the transition to hospice care.
Qualifications
- Active Registered Nurse (RN) license
- Graduate of an accredited nursing program (ADN or BSN)
- Valid driver’s license with reliable transportation
- Current CPR certification
- Strong clinical assessment and documentation skills
- Able to work independently and manage time effectively
- Excellent communication and interpersonal skills
- Able to provide compassionate care to patients and families
Preferred Qualifications
- Previous hospice, palliative care, or home health nursing experience
- Bachelor of Science in Nursing (BSN)
- Experience with admissions, care coordination, or case management
- Familiarity with electronic medical records (EMR) systems
- Knowledge of end-of-life symptom management
- Certification in hospice and palliative nursing (CHPN) or willingness to obtain
