RN Rapid Response Nights

About the role

As a Registered Nurse on the Rapid Response Team, you will oversee patient care using a professional practice model. Responsibilities include patient assessment, recognizing health issues, creating and evaluating care plans, and assessing patient responses. You will secure patient travel, address patient needs, and help prevent issues while adhering to the Texas Nursing Practice Act and delegating tasks based on team competencies.

Responsibilities

• Conduct detailed clinical assessments and prioritize patient and family needs.
• Create, implement, and evaluate care plans, adjusting them to achieve desired outcomes.
• Build therapeutic relationships with patients and families, managing discomfort and maintaining professionalism.
• Develop care plans that reflect patient values to aid in resolving issues.
• Educate patients, families, nurses, and community members to empower informed healthcare choices; assess and document learning needs regularly.
• Collaborate with colleagues and the community to create care plans, delegate tasks carefully, and follow up promptly.
• Use problem-solving strategies with patients, families, and staff, recognizing limitations to prioritize safety, effectiveness, and efficiency in care.
• Achieve goals through teamwork, recognize contributions, commit to growth, and support peers' development while demonstrating a commitment to community service.

Requirements

• Graduate of an accredited nursing program.
• Active RN license.
• BLS certification (must obtain within 30 days of hire if not already certified).

• Deep understanding of nursing and patient care standards, procedures, and regulations.
• Proficiency in medical terminology, including health, disease prevention, management, common medications, and their side effects.
• Respect for medical and professional nursing ethics and patient privacy rights.
• Exceptional verbal and written communication skills.
• Strong interpersonal skills to interact with a diverse range of individuals.
• Excellent problem-solving and critical-thinking abilities.
• Basic computer skills, including Microsoft Office, electronic medical documentation, information security, scheduling, payroll systems, and email.
